Sarah Hooper, JD is a Professor of Practice at UC Law San Francisco, where she has taught multidisciplinary teams about medical-legal needs of older adults with dementia for fifteen years. She has led or advised on development of clinical care models such as the Medical-Legal Partnership for Seniors, the Care Ecosystem model, and TRAC team at the San Francisco VA. She recently led a California statewide medical-legal learning collaborative in dementia with sixteen partners including UC health systems, safety net organizations, legal aid organizations and the Alzheimer’s Association.

Describe the benefits of proactively addressing legal and social care needs to patients, caregivers, and health systems
Describe the role of GUIDE teams in navigating these issues
